Preheat your oven to 350°F (175°C).
Heat oil in a skillet over medium heat. Add onion, bell pepper, mushrooms, and garlic. Sauté until soft, about 5-7 minutes. Remove from heat and let cool slightly.
In a large bowl, combine ground beef, breadcrumbs, milk, eggs, salt, pepper, Worcestershire sauce, and the sautéed veggies. Mix gently until combined.
Transfer the mixture to a loaf pan or shape into a loaf on a baking sheet.
Bake for 45 minutes. Remove from oven and sprinkle shredded cheese evenly on top.
Return to oven and bake an additional 10-15 minutes, until cheese is melted and meatloaf is cooked through.
Let rest for 5-10 minutes before slicing. Serve warm with ketchup or cheese sauce if desired.
