In a large skillet or saucepan, combine the water, Worcestershire sauce, onion powder, and garlic powder. Stir well to mix.
Add the frozen meatballs and sliced mushrooms to the pan.
Bring the mixture to a gentle boil over medium heat.
Sprinkle the brown gravy mix evenly over the simmering liquid, stirring continuously to avoid lumps.
Reduce heat to low and let the mixture simmer for 15–20 minutes, stirring occasionally, until the gravy thickens and the meatballs are heated through.
Taste and adjust seasoning if needed.
Serve hot with mashed potatoes, rice, or buttered noodles.
