Add the frozen pineapple, coconut milk, Greek yogurt, honey (if using), vanilla extract, and ice cubes to a high-speed blender.
Blend on high speed for 45–60 seconds, or until smooth and creamy.
Adjust the consistency by adding more frozen pineapple for a thicker smoothie, or a splash more coconut milk for a thinner texture.
Pour into a tall glass and serve immediately. Garnish with a slice of pineapple or a sprinkle of shredded coconut if desired.
