In a skillet over medium heat, melt the butter. Add the thinly sliced onions and cook slowly, stirring often, until golden and caramelized (about 20–25 minutes). Stir in the minced garlic during the last 2–3 minutes. Let cool slightly.

Prepare the Meatloaf Mix:
In a large bowl, combine ground beef, breadcrumbs, milk, egg, Worcestershire sauce, salt, and pepper.
Add half of the caramelized onions and half of the shredded cheese into the mixture. Mix gently until just combined — do not overwork the meat.

Shape and Assemble:
Form the meatloaf into a loaf shape on a lined baking sheet or in a loaf pan. Top with the remaining caramelized onions and sprinkle the rest of the cheese over the top.

Bake:
Preheat the oven to 350°F (175°C). Bake the meatloaf for 50–60 minutes, or until the internal temperature reaches 160°F (71°C).
